More about Brow and Forehead Lifts

As the brow and forehead ages, wrinkles begin to develop creating vertical “11s” between the eyebrows and horizontal wrinkles above the eyebrows on the forehead. Additionally, muscle tone and gravity can pull the eyebrows down and flatten their normal esthetic curvature. Generally speaking, the inner third of the eyebrow should be lower than the outer third to have an esthetic slope. In males the eyebrows are naturally lower than in females. Therefore changing the eyebrow shape or position can be performed to make patients look more masculine or feminine.

Significant drooping of the brow can weigh on the eyelids and give the eyes a tired appearance or even contribute to obstruction of vision. Sometimes correction of the brows can completely correct upper eyelid fullness, while in some cases eyelid surgery may also be needed. In minor cases, the brow and forehead can be treated using BOTOX Cosmetic to remove wrinkles and temporarily improve eyebrow position.

  • Brow lift: Ideal candidates for a brow lift are those with eyebrows that are too low or have lost their natural curve where the outer third is no longer in an elevated position compared to the inner third. The procedure can be performed either endoscopically (using small incisions behind the hairline) or open using a camouflaged incision at or behind the hairline, or directly within the eyebrow itself.
  • Transpalpebral corrugator resection: Ideal candidates for TPCR are those with strong vertical lines between the eyebrows (11s) and medial eyebrows that hang too low. The procedure involves a small incision made in the upper eyelid crease to remove the corrugator muscle group. These are the muscles responsible for bringing the medial eyebrow down and causing the vertical wrinkles. Often, a small amount of fat is placed to restore the muscle volume removed. This procedure is easily combined with an upper eyelid blepharoplasty if excess skin or bags of the upper eyelids also need correction.
  • Forehead lift: Ideal candidates for a forehead lift are those with deep horizontal wrinkles above the eyebrows and a long forehead. Excess forehead length may be due to sagging of the eyebrows as well as a receded hairline. The procedure involves repositioning the eyebrows, removing excess forehead skin, and reducing forehead wrinkles. In patients who need hairline correction as well, an incision at the hairline may be ideal, while incisions behind the hairline may be appropriate for those who need the forehead lifted along with the hairline. Combining this procedure with fat grafting or a laser treatment can further correct the appearance of wrinkles and complete the rejuvenation process.

Brow and Forehead Lift Recovery

After the operating room, you will be taken to a recovery area to be cared for. You will go home the same day or recover in a hotel if you have traveled in from out of town. You may have a mask or bandages that should be kept in place. Drains generally come out 1 to 2 days after surgery, while the mask and other bandages may be kept in place for 1 to 2 weeks.

Antibiotics are generally given on the day of surgery but not after surgery. We also prescribe pain medications to limit discomfort. Blood pressure medications may be added to minimize problems after surgery if your blood pressure ran high during the procedure or in recovery. You will receive after-care instructions before and after your surgery and have plenty of time to ask questions.

After surgery, you may have bruising, swelling, and soreness that will gradually go away. It is best to sleep with your head elevated on 2 or more pillows and not strain yourself for 2 weeks to reduce swelling. We may ask that you apply cold compresses to the area for 24 hours to further reduce swelling and bruising. Regular walking is encouraged to prevent blood clots, starting the morning after surgery.

We generally like to check on you in person the day after surgery. We will otherwise call and plan to see you in 8 to 10 days after surgery. We will schedule additional visits to check on your healing progress, which can be done virtually if you live out of town.

Healing takes time. Most patients feel comfortable socializing after 2 weeks. The bulk of the swelling resolves within a few weeks. Your face will continue to make gradual changes and your final results may take several months to be fully evident.
